手術後の「髄液漏れ」を防ぐために/前頭洞を安全に閉鎖する脳神経外科の工夫

脳動脈瘤や前頭蓋底腫瘍の手術では、両側前頭開頭という方法を用いることがあります。
この手術では、額の奥にある「前頭洞」という空洞が開放されることがあります。前頭洞は鼻の中とつながっているため、閉鎖が不十分だと、術後に髄液が鼻から漏れる「髄液鼻漏」や髄膜炎の原因になることがあります。私たちは、前頭洞の粘膜を丁寧に剥離し、6-0ナイロン糸で直接縫合して閉鎖する方法を行っています。さらにフィブリン糊をしみ込ませた素材で補強し、自家骨で保護することで、より確実な閉鎖を目指しています。
本研究では、前大脳動脈瘤に対して両側前頭開頭を行い、前頭洞が開放された34例を対象に、この方法の長期成績を検討しました。その結果、長期経過において髄液鼻漏や髄膜炎などの重篤な合併症を認めず、本手技の有用性が示されました。見えない部分の丁寧な処置こそが、安全な脳神経外科手術を支えています。

脳の血圧を直接測る / バイパス手術をより安全にするための挑戦

脳梗塞の危険が高い患者さんに対して行われるSTA-MCAバイパス術では、血管がつながったことは確認できても、「脳にどれくらい血液が届いているか」を直接評価する方法はありませんでした。私たちは上山博康先生が考案された、手術中に脳の血圧(中大脳動脈圧)を直接測定する方法を学びそのデータを詳細に調べました。その結果、術中の脳圧は術前SPECTで評価した脳血流とよく相関し、術後の過灌流症候群の予測にも有用であることを明らかにしました。脳循環を「見える化」することで、より安全で精度の高い脳血行再建術を目指しています。

くも膜下出血はなぜ日本人に多いか?:脳血管障害をきたす体内細菌叢の国際間比較研究
研究活動
日本医科大学脳神経外科では、脳卒中、脳動脈瘤、もやもや病、脳腫瘍などの脳神経外科疾患を対象として、基礎研究から臨床研究まで幅広い研究活動を行っています。
脳神経外科医は日々の診療や手術を通じて多くの課題に直面します。当教室では、その課題を研究によって解決し、患者さんへ還元することを使命と考えています。特に脳血管障害領域では、脳動脈瘤やもやもや病の発症機序の解明、くも膜下出血後の予後改善、新たなバイパス手術戦略の確立などを目指した研究を積極的に進めています。
近年は、もやもや病感受性遺伝子として知られるRNF213に着目し、もやもや病のみならず脳動脈瘤やその他の血管障害との関連について研究を進めています。また、くも膜下出血後の遅発性脳虚血障害や全身炎症反応に関する研究を通じて、重症脳卒中患者の予後改善を目指しています。
さらに、手術用顕微鏡、外視鏡、ヘッドマウントディスプレイなどの先進的な手術支援機器を用いた研究にも取り組み、安全性と術者の操作性向上を両立する次世代脳神経外科手術の開発を進めています。
これらの研究は、文部科学省科学研究費助成事業(科研費)をはじめとする競争的研究資金の支援を受けて実施されており、国内外の研究機関との共同研究を通じて推進されています。研究成果は国際学会や査読付き学術雑誌で積極的に発表し、基礎研究から臨床応用までをつなぐトランスレーショナルリサーチを実践しています。
今後も、世界水準の研究成果を創出するとともに、その成果を患者さんに還元することで、脳神経外科医療の発展と社会への貢献を目指してまいります。
